Case Management and Coordination
Our Resource Coordination Specialist helps you identify the services, supplies and assistance you need to care for your aging family member. Our staff will assist in developing a care plan that meets the individual needs of your family member to insure safety and well-being in the home. The Resource Coordination Specialist can guide you to the resources or access them on your behalf depending on your situation. A limited amount of free respite is available to give you a few hours or a few days of reprieve.
