Klein Older Adult Services now offers In-Home Care services as part of a new JFCS Concierge Program in Louisville, Kentucky.
The program is designed to promote independence and ensure safety for older adults in their home or alternative living situation. Client Care Managers will develop a Plan of Care with the family that best meets the individual’s needs. Arrangements will be made for caregivers to provide a variety of in-home services:
• Personal care and grooming
• Toilet/Incontinence Care
• Meal planning and preparation
• Eating Assistance
• Ambulatory Care
• Light housekeeping
• Laundry
• Transportation, Errands & Shopping
• Medication reminders
• Socialization and activity
• Cognitive Stimulation
• Respite
Our Care Managers will regularly visit the clients, allowing JFCS to evaluate and make any necessary changes to the client’s Plan of Care.
